When a child or youth is being referred to 4 DCFS a document will be filled out by the referring agency and will be used to assess the needs of the new resident.
How to Refer a Child or Youth
If you are a First Nation Child and Family Services Agency in Saskatchewan:
Email our main office at info@4dcfs.ca and ask for one of our referral packages:
- 0-11 yrs
- 12-15 yrs
4 DCFS Inc will send out one of our referral packages for your agency to fill out and questions that will be required.
